Please don’t send medical details through general web forms.
Our website and Google lead forms are intended for contact requests and scheduling—not for sensitive medical information.

Our practice phone number is provided through Google Voice. Standard telecommunications and carrier processes apply.

Note: We do not state that calls are recorded unless we explicitly notify you at the time of the call.

 

5) Lead forms (Google Ads)

If you submit your information through a Google Ads lead form, we receive the information you provide (typically name, email, and/or phone number) and may contact you to follow up.

Not for emergencies: Our online forms are not monitored 24/7. If you have an emergency, call 911 or go to the nearest emergency room.

 

6) How we share information

We do not sell your personal information.

We may share information with service providers who help us operate our website and communications, such as:

  • Wix (website hosting and website forms)

  • Google (GA4, Google Ads conversion tracking, Google Voice)

  • Call tracking providers (if used) for marketing attribution and forwarding

  • Charm/MedicalMine (practice management / EHR systems), when you become a patient or when used for scheduling/administration as applicable

We share information only as needed to provide services and operate our business.

 

7) HIPAA and patient information

If you become a patient, certain information is protected under HIPAA and is handled according to our Notice of Privacy Practices (separate from this website Privacy Policy).
